How Does the Plan Work?

The monthly cost is $100/month ($1200/year) per adult member billed quarterly (every 3 months). This is an annual, primary care membership plan with no office copays and no additional charges for annual physical exams, office follow-ups, telemedicine visits, messaging through secure electronic records to doctor and staff, phone calls by your provider, or completion of forms.  Many minor procedures and point of care tests are included.   Our membership numbers are kept small, so we can offer personalized care to our patients with same day or next day appointments available in most cases.  All of this is covered without additional expenses (see below).  For Families, we offer a discounted membership of $360 per year ($30 per month) to children age 6 to age 25 in school/college per paid adult parent which includes 6 office or tele-health visits per year.  We do not accept or bill insurance, but we can provide billing codes and visit receipts which you may be able to submit to your insurance for reimbursement. Membership fees may be covered by flexible spending and healthcare savings accounts.

It is our belief that a primary care physician should be an educator, motivator, and accountability partner with long range goals to decrease and prevent the burden of chronic disease, maintain an active quality of life, and hopefully work to decrease the use of prescription medications, as opposed to simply prescribing something to treat a symptom or side effect of a medication you might already be on. This is the main reason we developed a membership model for our primary care patients at Ultra Wellness Medical. Preventative health care and health optimization needs to be an ongoing process. Treating disease states such as hypertension, diabetes, heart disease and high cholesterol requires ongoing care and close monitoring. Patients are best served in a model that fosters frequent monitoring and support as opposed to periodic follow ups, as is often done today in many corporate primary care offices, where more time is spent by the provider staring at a screen, and writing a prescription, as opposed to providing personalized care to improve overall health, vitality, and make treatment options based on shared goals between the patient and physician.

What Is Included in Our Membership Plan?

Membership includes Routine Office visits plus 5 point of care services when needed per year, such as:

• Urinalysis
• Some Office Point of Care Testing as Available
• EKG
• Minor skin procedures such as freezing of benign lesions or abscess drainage when appropriate 
• Ear irrigation
• Minor wound care treatment

Members receive a comprehensive physical that includes a FIT3D body scan to assess precise body composition measurements and weight to provide insight into longterm health risks and to track goals over time. 

Our team does not administer immunizations in the office because we do not directly bill insurance, but we can assist you in getting the immunizations you need at outside facilities like pharmacies, etc. Our team works with LabCorp, Quest Diagnostics, hospitals and independent imaging companies for lab work and diagnostic testing. In addition, we offer specialty lab testing through Vibrant America.

Members receive discounts for the following in-office services:

• Emerald Laser - FDA Approved fat reduction and body sculpting.
• IV and IM injection nutritional therapy.
• Vitamin Supplement Pacs designed specifically by Dr. Williams for each patient's needs

As a member, you can discuss care by video or in person at the office. You have a patient portal in the electronic records system to message us 24/7.
